Keywords bring up 2 TNs one an image one a blank

A number of my keywords are bringing up pairs of thumbnails. One is of the actual image and the second is a blank of with the same metadata. Any clue as to what is happening here? Any way to clean it up?

Welcome to the forum. The "blank" TN represents an image that was imported and is now missing (note the small"!" in the upper right corner of the grid.) at some point you have reimported these images and now have two references to the same image file If you click on (!) for the "missing" image thumbnail, a message box will appear showing you where you originally imported that image

If you can report that path back here in a reply along with the path of the other copy of that image, we can probably advise on how to correct this. LR won't let you have two master image reference in the catalog that point to the same physical file.
 
Your thumbnails don't show file names, but the fact that you see one missing image and then one normal image each time, and the fact that the shape of the thumbnails is the same for each 'set', makes me believe that maybe you imported the same photos twice. Perhaps you moved the photos to a different folder (outside of Lightroom, so Lightroom didn't know and is now missing these), and then you imported this new folder into Lightroom.
